Congregate Nutrition Program

The Congregate Nutrition Program is available at various locations throughout the county, offering services from 1 to 5 days a week. This program includes food, fellowship, and various activities, such as:


  • Socialization
  • Exercise
  • Wellness education
  • Community support for the elderly, provided by site managers


Site managers actively engage in outreach and offer support to seniors in their respective areas.

Community Supportive Care

This program provides essential support to the elderly, addressing their needs and offering assistance in any area of concern. It includes site managers who visit seniors, arrange services, and ensure follow-up support. Key services provided include:

  1. Information and Referral
    Providing information and referral services to the elderly, their families, or friends. This helps link them to relevant resources and solutions.
  2. Benefit Specialist Services
    Offering legal and benefit advocacy, including assistance with government programs like Medicare, Social Security appeals, insurance counseling, and medical bill support.
  3. Handy-Helpers Chore Service
    Connecting seniors with individuals willing to perform various chores, including lawn work, snow shoveling, home maintenance, housekeeping, and companion services.
  4. Peer Counseling
    Trained volunteer peer counselors offering one-on-one assistance to individuals in need of consistent, ongoing support.
  5. Friendly Visitors
    Volunteers visiting homebound individuals regularly to provide companionship and social interaction.
  6. Telephone Reassurance
    Volunteers who call elderly individuals daily to check on their safety and offer limited socialization. If a person does not answer, follow-up procedures are initiated.
  7. Identification Cards
    Picture IDs for individuals 60 and older, enabling them to access senior discounts and providing additional identification.
  8. Recreation
    Funding for recreational programs for seniors, including activities like a bowling league, dartball league, bean-bag league, bus trips, and events at nutrition sites.

Home Delivered Program

Home-delivered meals are provided by Meals on Wheels and other local services. Areas covered include Sheboygan, Sheboygan Falls, Kohler, Plymouth, and Elkhart Lake. Volunteers from the Division on Aging deliver meals to those not covered by regular routes, including deliveries from Adell, Oostburg, and Plymouth Food and Fellowship sites.

Homespun Post

A craft outlet for senior citizens, allowing them to sell their crafts and generate income, which helps support their craft activities.

Eligibility Requirements Nutrition:

Program and direct services person or spouse must be 60 years of age. Information and referral any age

  • Program & Direct Services: The individual or their spouse must be 60 years of age or older.
  • Information & Referral: Available to individuals of any age.

National Family Caregiver Support Program

Provides assistance to caregivers, including:

  • Information and help obtaining services
  • Counseling
  • Funding for short-term respite services

State Senior Community Services Grant

This program offers volunteer opportunities for older adults, including:

  • Medical transportation (both in and out of the county)
  • Clerical and receptionist support at the office
  • Peer counseling
  • Assistance at nutrition sites and with home-delivered meals

Volunteers are reimbursed for meals and mileage.

State Alzheimer's Family & Caregivers Support Grant

Assists individuals caring for Alzheimer's patients. The grant provides:

  • Respite care
  • Adult day care
  • Support groups for caregivers

Widow/Widower Booklet

This booklet provides information and a checklist of tasks to complete after the death of a spouse. Site managers reach out to recent widows and widowers to offer assistance, if needed.

Wisconsin Parkinson's Association

Reimbursement for expenses related to the Parkinson's Support Group sponsored by the Office on Aging.
